Has anyone discovered the reason for the 3-ring releases at the Perrine a few weeks ago? Was it a rigging error or was it a manufacturing flaw?

Rumor on the street says it was a Basic Research Reactor.

I'm not trying to start any #####, but it is vital for the community to know one way or another what the cause or suspected cause was, because there are lives at stake whether it was a rigging error -or- a manufacturing flaw.

Does anyone have any useful, constructive info?
